Danation code isn't working

One out of 3 donation codes isn't working no matter what I tried - did everything by the book umpteen times! Even when generating new donation codes, still showing either service not available or STB is blocked. This is happening on 2 MAG254, 1 Dreamlink T1 and 1 Dreamlink T1 Plus. The other MAG254 and Dreamlink T1 with the remaining codes are working just fine. Any suggestions?
