I have an MXIII with Kodi and IPTV stalker on it. I've enter my Mac address that is registered with IKS66 but I don't see anywhere to enter my serial number, username or password. I downloaded the PDF but my settings looks a lot different in Kodi.

Your using the IPTV Stalker add-on ?
If so, isn't the serial number right under the mac address entry location ?
No user or pass required..just your mac address.

Hmm, maybe I don't have my serial number yet. They sent me an email and said to use the following as username and password. One of them is 13 numbers. I just figured that was the serial number.

The 13 digit number underneath username & password in the email is actually your route/donation number, so make sure you keep it in a safe place for future use.
If there's no box to enter a serial number there should be an option to send serial number' which sends a serial number & 'use custom serial number' which, when enabled will then display a box to enter a customised one that you have made up yourself, as well as device id's & signature. The same goes for entering your MAC address - you have to enable 'Custom MAC Address' in order for the box to enter your MAC address to be visible.
At this stage you may have corrupted your MAC so it's probably a good idea to change it. Go here:
https://iks66.tv/main/search
Enter your route/donation number (your route number is at the bottom of your confirmation email under Username & Password), complete captcha challenge & search. Go to bottom of page & click Change MAC. Then change MAC (use format 00:1A:78:XX:XX:XX) You can replace the X's with the last six digits of your android box or you can generate a new MAC by going here:

Got it working, thanks guys. One question, is there a way to stretch the picture on this? I don't like when my picture is boxed instead of stretched for my big screen.

Yes, you can do it by individual channel as you're watching by bringing up the OSD & clicking on the 'movie reel' icon & changing the view mode to stretch 16:9/Widezoom/Zoom etc. That will just change the view for that channel, but if you scroll down further you have the option to 'set as default for all videos'. Alternatively you can set the default video system settings in System>Video>Playback>Display 4:3 Videos As>Stretch 16:9/Widezoom/Zoom etc.
